Impact of Diabetes on Outcomes of Contemporary PCI Guided by OCT vs Angiography: The ILUMIEN IV Trial

The authors of the ILUMIEN IV trial aimed to evaluate how diabetes and complex coronary lesions affect outcomes in patients undergoing percutaneous coronary intervention (PCI) guided by optical coherence tomography (OCT) compared to angiography. They found that diabetic patients had significantly higher rates of target vessel failure and serious major adverse cardiovascular events after PCI, regardless of the guidance method used, with those having both diabetes and complex lesions at the highest risk. Ultimately, OCT guidance did not improve outcomes for either diabetic or nondiabetic patients.
