Effect of Hip Extension Compared With Usual Positioning on Perineal Lacerations During First Vaginal Delivery: A Randomized Clinical Trial

The authors aimed to determine whether hip extension during crowning in first vaginal deliveries reduces the incidence of significant perineal lacerations and obstetric anal sphincter injuries compared to usual positioning. The study found no significant difference in the primary outcome of significant lacerations between the two groups, although some unadjusted analyses suggested potential benefits of hip extension. Ultimately, the conclusion was that hip extension did not lead to a reduction in significant perineal lacerations in nulliparous patients.
