The authors aimed to reassess the diagnostic criteria for acute kidney injury (AKI) in neonates with hypoxic-ischaemic encephalopathy undergoing therapeutic hypothermia, comparing the modified neonatal KDIGO (nKDIGO) criteria with the Gupta definition. Their findings indicated that the Gupta definition identified additional cases of AKI that were missed by nKDIGO, and both definitions were associated with a greater need for inotropic support and higher cystatin C levels in early adolescence among affected infants. Incorporating the Gupta criteria could enhance the detection of clinically significant AKI in this vulnerable population.
