Ultra-Widefield OCT Mapping of Foveal and Optic Nerve Head Topography Within Posterior Staphyloma in Pathologic Myopia

The authors aimed to define the spatial relationship between the fovea and optic nerve head (ONH) in eyes with pathologic myopia and posterior staphyloma, and to evaluate their associations with foveal complications and peripapillary atrophy. Using ultra-widefield optical coherence tomography (UWF-OCT), they found that the fovea is more often located on the slope of the staphyloma, while ONH location is influenced by axial length, with longer eyes less likely to have the ONH at the staphyloma bottom. Additionally, peripapillary atrophy was associated with both ONH location and axial length, indicating distinct remodeling mechanisms in the macula and peripapillary region in pathologic myopia.
