A urothelial cell carcinoma in the abdominal cavity after salpingectomy for tubo-ovarian carcinoma risk reduction in a BRCA1 pathogenic variant carrier.

This case report describes a 37-year-old BRCA1 pathogenic variant carrier who developed extensive abdominal carcinomatosis two years after risk-reducing salpingectomy with delayed oophorectomy, initially appearing ovarian in origin. Pathology revision ultimately classified the tumor as a poorly differentiated sarcomatoid urothelial cell carcinoma, carrying the BRCA1 variant. The authors use this case to highlight the need for structured clinical trial settings to safely evaluate alternative prevention strategies, with close follow-up and thorough pathological reassessment.
