Clinical Implications of Transmural Late Gadolinium Enhancement in Genotype-Positive Arrhythmogenic and Dilated Cardiomyopathy

The authors aimed to investigate the prevalence, genetic associations, and prognostic significance of transmural late gadolinium enhancement (LGE) in patients with genotype-positive, nonischemic arrhythmogenic and dilated cardiomyopathy. They found that transmural LGE, present in 5% of the studied cohort, was linked to higher rates of major ventricular arrhythmic events and advanced heart failure therapies, particularly among patients with specific genetic variants (LMNA and DSP), indicating it identifies a high-risk subset within this population.
