Comparison of SWI with T2* in the Diagnosis of Cerebral Amyloid Angiopathy Using ARIA Scoring and Boston Criteria 2.0 at 7T MRI

This study aimed to compare the diagnostic performance of T2* and susceptibility-weighted imaging (SWI) for detecting cerebral amyloid angiopathy (CAA) at 7T MRI, as well as to assess the applicability of the Boston Criteria 2.0 in this context. The results indicated that both imaging techniques demonstrated almost perfect agreement in identifying hemorrhagic markers associated with CAA, while high interobserver agreement was observed, supporting the reliability of ARIA-based scoring. However, the detection of deep microbleeds may lead to misclassification, suggesting a need for updated diagnostic frameworks for 7T MRI.
